The most common cause of true sciatica — when the inner material of an intervertebral disc protrudes and presses on the nerve root exiting the spine. Even a relatively small disc bulge at the L4/5 or L5/S1 level can cause significant sciatic symptoms.

Facet Joint Dysfunction

Inflamed or restricted facet joints at the lumbar spine can irritate the adjacent nerve roots, causing referred pain and sciatic-like symptoms down the leg.

Piriformis Syndrome

The sciatic nerve runs directly beneath — or in some people, through — the piriformis muscle in the buttock.
